This Owner Surrender Agreement (“Agreement”) is entered into between The Last Resort Animal Sanctuary (“Sanctuary”) and the undersigned individual (“Owner”).

By signing this document, the Owner irrevocably surrenders ownership of the animal described below to the Sanctuary.

SURRENDER TERMS

1. Ownership Transfer

Owner hereby transfers all rights, title, and interest in the animal to The Last Resort Animal Sanctuary. From the date of signing, the Sanctuary has full ownership and control.

2. Surrender Fee

Owner agrees to pay a $35 surrender fee, which helps cover intake, veterinary care, and shelter costs.

Payment accepted: Cash or Credit Card.
Fee is non-refundable.

3. Health & History

Owner affirms that they have provided full disclosure of the animal’s medical history, behavior, and any known issues.

4. No Reclaim

Once surrendered and fee is paid, the animal cannot be reclaimed by the Owner.

5. Sanctuary Rights

The Sanctuary may provide medical care, adopt, foster, or humanely manage the animal in its discretion.

ACKNOWLEDGMENT

I, the undersigned Owner, have read, understand, and agree to all terms of this Surrender Agreement. I am voluntarily surrendering this animal and transferring ownership to The Last Resort Animal Sanctuary.
